Answers: Those are "generically" # 894 Bulbs,,it's in recent times a fog light bulb
It's a 800 Series Bulb,,,, a T-3 1/4 Lamp beside a
PGJ12 Right Angle Base.
The PG13 is a Straight Base version,,,and is characterized 880,893,899 etc according to Watts and Life
(They're Interchangeable,,,sometimes it takes SLIGHT modification---trim the locking tabs)
899 are rate at 10X the life,,,2,000hrs vs 200hrs
Most auto parts should hold them,,bring your old bulb beside you & they can help clash it.
It's Possible that even WalMart may have them surrounded by AutoParts section??
I never looked for them near,,so I'm not certain.
But it's NOT a unusual bulb,
Easy to identify by the sorta Unique appearance of the Globe standing Off the Base,,,mounted on the filament pins.

Here's one on ebay

Here's a cheaper one

Here's a BUNCH on Ebay,,,adjectives sorts of colors & Hype ,etc.
BEST SUGGESTION,,stick with a "Normal" Name Brand OEM Type.
The Snazzy ones are crap,,,,bulb life span is short enough as-is on 800 Series lamp

Maybe adjectives in your hunt to write down they list application as:
Fits Polaris:
Sportsman 500 4x4 (All Variations) (96)
Sportsman 700 4x4 (05)
Sportsman 700 EFI (05)
Sportsman 700 MV7 (05)
Sportsman 800 EFI (05)
Xplorer 400L 4x4 (95 & 96)

The Relay which controls Hi/Lo Beam switching is NOT surrounded by the Bulb.
It's an external part,,,controlled by Switch.
As You select Beam Choice ,,
The Switch triggers the appropriate contact set within the Relay,,,which in turn sends electricity to the Bulbs as expected
Nothing "special" or Proprietary about the bubs,,,
they're simply Fog Lamp bulbs
.........................................
IF You wanna fool with it,,
They are Tuff to find "locally" but I'm NOT Kidding something like the Life of the 899 vs the other 800's
Sylvania:
http://www.sylvaniaautocatalog.com/sylva...
Thing is,,,they are STRAIGHT BASE,
rather than Right Angle close to your original 894's
Other than That,,they are "same"(supposed to be Direct Fit--sometimes require Minor trimming)
The Biggest "Catch" is SPACE.
Bulb's Straight Base itself is Barely longer,,but when ya include Wiring Connector plugged into It,,,
It's Way Long.
Thus,,,wont fit into a Shallow Lamp Shell w/o some ,,Uh,,,creativity.
The Trade-off for the dramatic increase contained by Life is a bit Dimmer Light Output.
Not Much less,,,but it IS smaller number.
But NOT as Dim as a burned-out/broke filament 100 hour bulb.
One other thing,,,,
only a mention for general info's sake.
You innovative bulbs are 37.5watts.
That's "standard wattage" for 800 series.
There ARE some 50W ones available,,,,but they're all "low life" bulbs,,and Possibly too much electrical nouns for your system.

Here's Pics of the Bases,,,Straight & Right Angle.
If You can Determine the amount of Space/Room ya have contained by the Headlight,,,,
You MIGHT be able to shop for Either Bulb,,,and plug-&play.
